Information on registration for COVID-19 Vaccination

In connection with numerous calls from the University students, we would like to inform that the registration form has been once again activated and can be accessed at:
https://forms.sum.edu.pl/formularz/pUDfhx
The form can be used exclusively by persons who failed to register for Covid -19 immunization before 14 January 2021.
Pursuant to the Announcement of the Ministry of Health, which was addressed to the vaccination points, the Group 0 immunization has been scheduled until 21 March 2021. In consequence, the form will remain active only until 17 March 2021.
For organizational reasons, persons who have registered via the above named University form will be referred solely to the University’s clinical hospitals for immunization.
